And why does it matter?<\/p>\n<h3>From use to abuse<\/h3>\n<p><strong>Substance use could be seen as a spectrum. At one end<\/strong>, you have someone who uses no substances. At the other, addiction. In the middle, people who use substances, but it\u2019s not negatively affecting their life.<\/p>\n<p><strong>Substance abuse could be seen as the space between <\/strong>the middle and the end. Between using without issue, and addiction.<\/p>\n<p><em>It means you are using substances in ways that are increasingly having a negative impact on your physical or <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/what-is-mental-health.htm\">psychological health<\/a>, even if you don\u2019t qualify as an addict.<\/em><\/p>\n<h3>What sort of substances are we talking about?<\/h3>\n<p><strong>Don\u2019t have a problem, because you are just using a<\/strong> few extra painkillers here and there, but they are your prescription? Or because it&#8217;s \u2018only wine\u2019?<\/p>\n<blockquote><p><span style=\"color: #800080;\">It doesn\u2019t have to be <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/club-drugs-and-therapy.htm\"><span style=\"text-decoration: underline;\">street drugs<\/span><\/a>\u00a0to be an issue. Misuse of prescription medication is also substance abuse. And wine is alcohol. <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/alcohol-and-depression.htm\"><span style=\"text-decoration: underline;\">Alcohol abuse<\/span><\/a> is a common and very real problem.<\/span><\/p><\/blockquote>\n<p><strong>And yes, smoking weed counts, too. Here in the UK, the<\/strong> National Institute for Health and Care Excellence (NICE) states in their <a href=\"https:\/\/www.nice.org.uk\/guidance\/cg51\/chapter\/Introduction\">guide to drug misuse<\/a> that cannabis is the most used drug, followed by cocaine, then other stimulants such as amphetamine.<\/p>\n<h3>Official diagnoses of substance abuse and addiction<\/h3>\n<p><strong>Is substance abuse an official mental health diagnosis? Yes<\/strong> &#8212; but the wording has recently changed, and depends on what reference guide your mental health professional is using.<\/p>\n<h3>DSM-V definitions<\/h3>\n<p>In America, diagnoses are all done with the <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/what-is-the-dsm.htm\">Diagnostic and Statistical Manual of Mental Disorders (DSM-5)<\/a>.<\/p>\n<p><strong>Until recently, you could receive an official diagnosis of &#8216;substance abuse disorder&#8217; or<\/strong> &#8216;substance dependence disorder&#8217; (the official term for addiction).<\/p>\n<blockquote><p><span style=\"color: #333399;\">But the current edition, the DSM-5, has combined abuse disorder and dependence disorder into one category, &#8216;<strong>substance use disorder&#8217;,<\/strong> that works over a spectrum. You can have either a mild, moderate, or severe version.<\/span><\/p><\/blockquote>\n<p><span style=\"color: #333333;\"><strong>This is determined by how many symptoms you have from a list of eleven.<\/strong> T<\/span><span style=\"color: #333333;\">wo or three would indicate \u2018mild substance use disorder\u2019, four or five symptoms \u2018moderate\u2019, and six or more \u2018severe\u2019.<\/span><\/p>\n<p><strong>The list of symptoms includes:<\/strong><\/p>\n<ul>\n<li>using in greater amounts and for a longer time that originally intended<\/li>\n<li>wanting to stop or cut back, but not actually managing to<\/li>\n<li>spending a lot of time on your finding, using, and recovering from substance use<\/li>\n<li>thinking about using the substance when you aren\u2019t (craving)<\/li>\n<li>no longer maintaining your<a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/commitment-issues-signs.htm\"> life commitments<\/a> because of use<\/li>\n<li>using even when it\u2019s messing up your<a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/healthy-relationships-is-it-one.htm\"> relationships<\/a><\/li>\n<li>giving up activities that were important to you because of use<\/li>\n<li>using even when you are putting yourself in harm\u2019s way<\/li>\n<li>and even if you know you are physically and mentally deteriorating because of use<\/li>\n<li>needing more and more of the substance to get the same effect<\/li>\n<li>experiencing physiological withdrawal symptoms.<\/li>\n<\/ul>\n<h3>World Health Organisation ICD-10 definitions<\/h3>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignright size-medium wp-image-120090\" src=\"https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/goashape-QzWzLokHvxE-unsplash-400x266.jpg\" alt=\"substance abuse vs addiction\" width=\"400\" height=\"266\" srcset=\"https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/goashape-QzWzLokHvxE-unsplash-400x266.jpg 400w, https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/goashape-QzWzLokHvxE-unsplash-500x333.jpg 500w, https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/goashape-QzWzLokHvxE-unsplash-180x120.jpg 180w, https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/goashape-QzWzLokHvxE-unsplash.jpg 709w\" sizes=\"auto, (max-width: 400px) 100vw, 400px\" \/>The World Health Organisation and their manual the <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/what-is-the-icd-10.htm\">International Classification of Diseases (ICD-10)<\/a>, is used outside of America (including here in the UK).<\/p>\n<blockquote><p><span style=\"color: #333399;\">They <a style=\"color: #333399;\" href=\"https:\/\/www.who.int\/classifications\/icd\/en\/GRNBOOK.pdf?ua=1\"><span style=\"text-decoration: underline;\">classify things differently<\/span><\/a>, not using the term substance abuse or addiction\u00a0 but referring to \u2018<strong>harmful use<\/strong>\u2019 and \u2018<strong>dependence syndrome<\/strong>\u2019.<\/span><\/p><\/blockquote>\n<p><strong>Harmful use (previously called \u2018non dependent use\u2019) means<\/strong> that your intake of a substance \u201cwas responsible for (or substantially contributed to) physical or psychological harm, including impaired judgement or dysfunctional behaviour&#8221;.\u00a0 This needs to have gone on for at least one month, or have occurred repeatedly within a twelve-month period.<\/p>\n<p><strong>Dependence syndrome involves<\/strong> other symptoms. You must have had three or more of the following, again, for at least a month, or on and off over a 12 month period.<\/p>\n<ul>\n<li>a strong desire or sense of compulsion to take the substance<\/li>\n<li>losing the ability to control your substance taking, including starting, stopping, amounts of substance, time you keep using<\/li>\n<li>physiological withdrawal symptoms if you reduce or stop using the substance, including taking the substance again to avoid these symptoms<\/li>\n<li>developing a tolerance for the substance, so you need to take increasing amounts to achieve the same effect<\/li>\n<li>becoming preoccupied with the substance, meaning you stop doing other things that used to give you pleasure, spend a lot of time figuring out how to get your substance, and then a lot of time recovering<\/li>\n<li>continuing to use the substance even though it\u2019s clear it\u2019s harming you and your life.<\/li>\n<\/ul>\n<blockquote><p><span style=\"color: #800080;\">On their website, <a href=\"https:\/\/www.who.int\/substance_abuse\/terminology\/abuse\/en\/\"><span style=\"text-decoration: underline;\">the WHO also points out<\/span><\/a> that with harmful use, the physical and\/or psychological harm does not have to have social consequences, whereas with dependence it generally does.<\/span><\/p><\/blockquote>\n<h3>National Institute for Health and Care Excellence (NICE)<\/h3>\n<p><strong>The National Institute for Health and Care Excellence (NICE)<\/strong> is a British organisation that puts out guidelines to diagnosis and treatment that are referred to by UK health care workers.<\/p>\n<p><strong>They refer to &#8216;drug misuse&#8217; over &#8216;abuse&#8217;.\u00a0<\/strong>With alcohol they use the term &#8216;alcohol use disorders&#8217;.<\/p>\n<h3>Substance abuse vs Addiction<\/h3>\n<p><em>Looking at these different approaches, what conclusions can we draw about substance abuse vs addiction?<\/em><\/p>\n<p><strong>Both mean that<\/strong> a substance is negatively affecting your life, and that it&#8217;s been going on for a month non stop, or over the course of a year.<\/p>\n<p><strong>Both abuse and addiction mean you continue using a substance<\/strong> even though it&#8217;s clear it&#8217;s becoming harmful.<\/p>\n<p><strong>But with addiction, you experience<\/strong> increasing tolerance, withdrawal, can&#8217;t control your use, are more and more preoccupied, and continue to use even though it&#8217;s <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/lying-in-relationships.htm\">damaging your relationships<\/a>.<\/p>\n<h3>So why do the differences matter?<\/h3>\n<p><strong>It can be all too easy to tell yourself you don&#8217;t have a problem with your substance of choice as you &#8216;aren&#8217;t addicted&#8217;.<\/strong> We make it about &#8216;control&#8217;. But as the above illustrates, if a substance<em> is in any way affecting your life negatively, physically or mentally<\/em>, then there is a problem. This can look like:<\/p>\n<ul>\n<li>a few glasses of wine every night meaning you are always tired and <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/bad-parents-are-you-one.htm\">snap at your children<\/a><\/li>\n<li>using more painkillers at work than your doctor advised and it&#8217;s affecting your performance<\/li>\n<li>having constant nose bleeds from cocaine use<\/li>\n<li><a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/psychology-of-food.htm\">gaining a lot of weight<\/a> due to cannabis use<\/li>\n<li><a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/mental-wellbeing.htm\">sabotaging a promising new relationship<\/a> because they feel you drink or use drugs too much.<\/li>\n<\/ul>\n<p><strong>Also note that you might be using more than one substance, and facing up to a problem can involve looking at the big picture.<\/strong> If you only focus on your cocaine use, not factoring in the several times a week you smoke cannabis, it can be far easier to pretend there isn&#8217;t a problem when there is.<\/p>\n<p><strong><span style=\"color: #800080;\">Time to face up to a substance abuse problem and seek help?
