There are now far more options to choose from \u2014 if you live in the right country.<\/p>\n<p><strong>Europe, for example, prescribes less medications<\/strong> than the leader of ADHD drugs, America. The UK falls in between Europe and the USA, with a decent choice on offer, if not everything on the market.<\/p>\n<blockquote><p><span class=\"s1\" style=\"color: #333399;\">If you would like to try ADHD medication in the UK, then you must go see a <a href=\"https:\/\/www.harleytherapy.co.uk\/adult-attention-deficit-hyperactivity-disorder-treament-london.htm\">psychiatrist<\/a>. They are the only mental health professional who can give you an official diagnosis (should you need paperwork for <a href=\"https:\/\/www.harleytherapy.co.uk\/insurance.htm\">insurance<\/a> or otherwise) and then prescribe medication.<\/span><\/p><\/blockquote>\n<p>[Watch Dr. Stephen Humphries, ADHD specialist psychiatrist in London, <a href=\"https:\/\/www.youtube.com\/watch?v=dWD2C5xL9IQ\" target=\"_blank\" rel=\"noopener\">discuss ADHD meds in the UK here<\/a>].<\/p>\n<h2>Licensed vs unlicensed ADHD meds<\/h2>\n<p><strong>A &#8216;licensed medication&#8217; means that it has<\/strong> gone through research trials that were approved by a regulatory body here in the UK.<\/p>\n<p><strong>An &#8216;unlicensed medication&#8217; can refer to a medication that <\/strong>has not been fully tested. More often than not, though, it means a medication that is an import. Or, that a medication has been found to work for various issues beyond its original intention. So the maker has decided it&#8217;s too expensive to license it for everything and has made it &#8216;off license&#8217;.<\/p>\n<blockquote><p><span style=\"color: #333399;\">When it comes to ADHD meds, an &#8216;unlicensed medication&#8217; usually just means that the medication is licensed in other countries but not here in the UK, so is imported. And they can be short or long acting,<\/strong> also called \u2018immediate release\u2019 and \u2018modified release\u2019. <\/span><span class=\"s1\" style=\"color: #333333;\">If you are nervous about trying ADHD medications you might want to ask for a short-acting medication which can give you an idea of what it&#8217;s like to be on meds.\u00a0<\/span><\/p>\n<p>The two types of stimulants for ADHD are the <em>methylphenidate <\/em>family (such as the infamous Ritalin), and the <em>amphetamine salts<\/em> family (like Adderall).<\/p>\n<h2 class=\"p1\"><span class=\"s1\">Methylphenidates<\/span><\/h2>\n<p class=\"p1\"><span class=\"s1\"><strong>These are the most prescribed medications across Europe and the UK.<\/strong> The full scientific name is &#8216;<a href=\"https:\/\/www.nice.org.uk\/bnf-uk-only\">methylphenidate hydrochloride<\/a>&#8216;. When short acting they last around two to three hours. <\/span><\/p>\n<p class=\"p1\"><span class=\"s1\"><strong>Ritalin, the most famous\u00a0 methylphenidate of all, is<\/strong> available in the UK, but <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/add-symptoms-in-children.htm\">only licensed for children<\/a> over six and not for adults.\u00a0<\/span><\/p>\n<p class=\"p1\"><strong><span class=\"s1\">Licensed choices available are:<\/span><\/strong><\/p>\n<ul>\n<li><span class=\"s1\">generic methylphenidate\u00a0<\/span><\/li>\n<li class=\"p1\"><span class=\"s1\">Concerta XL\u00a0<\/span><\/li>\n<li>Delmosart prolonged release<\/li>\n<li class=\"p1\"><span class=\"s1\">Eqasym XL<\/span><\/li>\n<li class=\"p1\"><span class=\"s1\">Medikinet XL<\/span><\/li>\n<li class=\"p1\"><span class=\"s1\">Xaggitin XL.<\/span><\/li>\n<\/ul>\n<p><strong>Note that doses of a methylphenidate medication of over 60\u202fmg daily are not licensed<\/strong>, and with Concerta XL the limit is 54mg.<\/p>\n<h3><span style=\"color: #333399;\">Side effects of methylphenidate ADHD medications<\/span><\/h3>\n<p class=\"p1\"><span class=\"s1\">These include an increased heart rate and blood pressure. <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/sleep-problems-mental-health-issue.htm\">Sleep problems<\/a>, stomach upset, headaches, appetite loss, teeth grinding or jaw clenching. Possibly the return of physical tics you had in the past, and shifts in your moods including agitation. Very rarely they can cause <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/depression-with-psychosis.htm\">psychosis<\/a>. <\/span><\/p>\n<h2 class=\"p1\"><span class=\"s1\">Amphetamine salts<\/span><\/h2>\n<p>Amphetamine salts contain either <span class=\"s1\"><strong>dexamfetamine<\/strong> (sometimes spelled &#8216;dexamphetamine&#8217;), or &#8216;<strong>lisdexamfetamine<\/strong>&#8216; (full scientific name &#8216;<a href=\"https:\/\/www.nice.org.uk\/bnf-uk-only\">lisdexamfetamine mesilate<\/a>&#8216;),\u00a0 which is a mixed salt.<\/span><\/p>\n<blockquote><p><span class=\"s1\"><span style=\"color: #800080;\">\u00a0Lisdexamfetamine is licensed in the UK. If you are prescribed but do not do well with either this or methylphenidates, then you will still be offered generic dexamfetimine.<\/span><\/span><\/p><\/blockquote>\n<div id=\"attachment_124043\" style=\"width: 385px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-124043\" class=\"size-medium wp-image-124043\" src=\"https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/3100508753_dd00ffec98-e1667894966240-375x400.jpg\" alt=\"ADHD medication\" width=\"375\" height=\"400\" srcset=\"https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/3100508753_dd00ffec98-e1667894966240-375x400.jpg 375w, https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/3100508753_dd00ffec98-e1667894966240.jpg 400w\" sizes=\"auto, (max-width: 375px) 100vw, 375px\" \/><p id=\"caption-attachment-124043\" class=\"wp-caption-text\">By: <a href=\"https:\/\/www.flickr.com\/photos\/arenamontanus\/3100508753\/\" target=\"_blank\" rel=\"nofollow noreferrer\">Anders Sandberg<\/a><\/p><\/div>\n<p><strong><span class=\"s1\">Off license options available are:<\/span><\/strong><\/p>\n<ul>\n<li class=\"p1\"><span class=\"s1\">Adderall (only available as extended release, the\u00a0 instant release version popular in America is not available).\u00a0<\/span><\/li>\n<\/ul>\n<p><strong>Note that lisdexamfetamine is not prescribed if<\/strong> you have a<a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/what-is-comorbidity.htm\"> dual diagnosis<\/a> of <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/bipolar-disorder.htm\">bipolar disorder<\/a> or <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/substance-abuse-vs-addiction.htm\">substance abuse issues<\/a>.<\/p>\n<p><strong>Also note that many European countries do not prescribe amphetamine salts. They are considered illegal<\/strong> drugs! So if you travel, be very certain you take a doctor&#8217;s letter with you!<\/p>\n<h3><span style=\"color: #333399;\">Side effects of amphetamines<\/span><\/h3>\n<p class=\"p1\"><span class=\"s1\" style=\"color: #333333;\">Side effects wise, note that amphetamines can cause drowsiness and dizziness instead of the buzz of methylphenidates. And you might also deal with nausea and diarrhoea.<\/span><\/p>\n<h2 class=\"p1\"><span class=\"s1\">Non stimulants<\/span><\/h2>\n<p>There is one non stimulant commonly prescribed amongst ADHD medications in the United Kingdom and Europe, which is Atomoxetine.<\/p>\n<h4 class=\"p1\"><span class=\"s1\">Atomoxetine<\/span><\/h4>\n<p class=\"p1\"><span class=\"s1\"><strong>This is a selective re-uptake inhibitor (SNRI), so a very different sort of medication.<\/strong> It increases noradrenaline in the brain, a sort of messenger chemical passing information. The end result is better concentration as your impulses are lowered. Note that unlike other ADHD meds that are fast-acting, it also takes several weeks to have effect, much like an anti-depressant would.\u00a0<\/span><\/p>\n<h3><span style=\"color: #333399;\">Side effects of antomoxetine<\/span><\/h3>\n<p class=\"p1\"><span class=\"s1\">Side effects can include stomach cramps and diarrhoea. It has a possible dangerous side effect which is <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/managing-suicidal-thoughts.htm\">suicidal thinking<\/a> or<a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/rise-in-self-harming.htm\"> a desire to self-harm<\/a>. And it can be hard on your liver. For these reasons, it&#8217;s usually only prescribed after you&#8217;ve tried stimulants and they didn&#8217;t work for you.\u00a0<\/span><\/p>\n<h2 class=\"p1\"><span class=\"s1\">Other unlicensed options<\/span><\/h2>\n<p><span class=\"s1\"><strong>Briefly, it is possible for a UK psychiatrist to treat adult ADHD with:<\/strong><\/span><\/p>\n<ul>\n<li><span class=\"s1\"> Modafinil<\/span><\/li>\n<li><span class=\"s1\">the antidepressant Venlafaxine<\/span><\/li>\n<li><span class=\"s1\"> bupropion hydrochloride<\/span><\/li>\n<li><span class=\"s1\">tricyclic antidepressants. <\/span><\/li>\n<\/ul>\n<p class=\"p1\"><span class=\"s1\">But there are all considered unlicensed ADHD medications. They don\u2019t come recommended by the NHS or the UK\u2019s diagnostic advice body, the National Institute for Health and Care Excellence (NICE)<\/span><\/p>\n<h2 class=\"p1\"><span class=\"s1\">ADHD medications on the NHS <\/span><\/h2>\n<p class=\"p1\"><strong><span class=\"s1\">Of the above medication choices, an NHS practitioner will offer you:<\/span><\/strong><\/p>\n<ul>\n<li class=\"p1\"><span class=\"s1\">methylphenidate<\/span><\/li>\n<li class=\"p1\"><span class=\"s1\">dexamfetamine <\/span><\/li>\n<li class=\"p1\"><span class=\"s1\">lisdexamfetamine<\/span><\/li>\n<li class=\"p1\"><span class=\"s1\">antomoxetine.<\/span><\/li>\n<\/ul>\n<p>And they would follow the NICE guidelines.<\/p>\n<h2 class=\"p1\"><span class=\"s1\">What does NICE recommend?<\/span><\/h2>\n<p class=\"p1\"><span class=\"s1\"><strong>In America, mental health professionals all refer to<\/strong> the DSM when diagnosing (<a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/what-is-the-dsm.htm\">the Diagnostic and Statistical Manual of Mental Disorders<\/a>). Other countries refer to <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/what-is-the-icd-10.htm\">the ICD,<\/a>\u00a0the guide put out by the World Health Organisation (WHO). <\/span><\/p>\n<blockquote>\n<p class=\"p1\"><span class=\"s1\" style=\"color: #800080;\">Here in the UK diagnostic guides are put out by the National Institute for Health and Care Excellence (NICE).<span class=\"Apple-converted-space\">\u00a0 <\/span>They take into account both the DSM and ICD and all current and up-to-date research and evidence to make their recommendations for UK practitioners. <\/span><\/p>\n<\/blockquote>\n<p class=\"p1\"><span class=\"s1\">The current <a href=\"https:\/\/www.nice.org.uk\/guidance\/ng87\">NICE advice for treatment of adults diagnosed with ADHD<\/a> is as follows: <\/span><\/p>\n<ol class=\"ol1\">\n<li class=\"li1\"><span class=\"s1\">Adults should first be offered lisdexamfetamine or methylphenidate. <\/span><\/li>\n<li class=\"li1\"><span class=\"s1\">If after six weeks of methylphenidate the benefits aren\u2019t showing then switch to lisdexamfetamine , and vice versa. <\/span><\/li>\n<li class=\"li1\"><span class=\"s1\">If an individual has tried lisdexamfetamine and it\u2019s helping symptoms, but a client finds the long duration of the effects hard to tolerate, then offer dexamfetamine. <\/span><\/li>\n<li class=\"li1\"><span class=\"s1\">Offer atomoxetine only if the client can\u2019t tolerate<span class=\"Apple-converted-space\">\u00a0 <\/span>lisdexamfetamine or methylphenidate, or neither has proved much result.\u00a0<\/span><\/li>\n<\/ol>\n<h2 class=\"p1\"><span class=\"s1\">Things to consider when deciding on ADHD medication in the UK<\/span><\/h2>\n<ul>\n<li>Do I already know I focus and perform better when stimulated, like when I drink a lot of coffee? Or when I am more relaxed, like when I am a bit tired or have <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/meditation-and-the-brain.htm\">meditated<\/a>?<\/li>\n<li><span class=\"s1\">Do I want to take it every day, or just on work days? (Some of these medications are dangerous to just stop and have to be tapered).<\/span><\/li>\n<li>Do I want to be medicated just part of the day, or for most of the day?<\/li>\n<li>Are there\u00a0 any pre-existing or prior medical conditions I need to consider and discuss?<\/li>\n<li>Is my medication of choice something I can get through the system, or do I need to book a private psychiatrist?<\/li>\n<\/ul>\n<h2>And most important of all when it comes to ADHD treatment<\/h2>\n<p class=\"p1\"><span class=\"s1\"><strong>It&#8217;s important to note that taking medication is a personal choice. You do not not have to take<\/strong> meds, and no psychiatrist should ever pressure you to do so.\u00a0 Yes, medications are the first line of treatment here in the UK for adults (note this is not the case for <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/adhd-in-children-should-you-be-concerned.htm\">children<\/a>) but they are not the only way forward.\u00a0<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\"><a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/adult-adhd-treatment.htm\">Talk therapy is generally recommended for adult ADHD<\/a> as well. This is because ADHD medications only manages symptoms and doesn&#8217;t change longstanding negative coping mechanisms.<span class=\"Apple-converted-space\">\u00a0 <\/span>And some people decide to just go the therapy route and decide on medication later, or decide to never take meds.<\/span><\/p>\n<p><strong><span style=\"color: #333399;\">Need a diagnosis or treatment for adult ADHD?
