And have you been starting to feel surreal, or had strange thoughts lately? Worried you have depression with psychosis? Or that it&#8217;s <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/schizophrenia.htm\">schizophrenia<\/a>?<\/p>\n<h2>What is depression with psychosis?<\/h2>\n<p><span style=\"font-weight: 400;\">Depression<\/span><span style=\"font-weight: 400;\"> with psychosis is considered a form of <\/span><span style=\"font-weight: 400;\">major depression,<\/span><span style=\"font-weight: 400;\"> and is essentially severe depression with some symptoms of psychosis. These symptoms can include <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/flashback-vs-hallucination-vs-delusion.htm\">hallucinations<\/a> (sensory experiences not based on reality) and delusions (mistaken beliefs based on imagination rather than reality).\u00a0<\/span><\/p>\n<h3>Symptoms of depression with psychosis<\/h3>\n<p><span style=\"font-weight: 400;\"><strong>You will experience a combination of depression symptoms,<\/strong> which could include:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">low mood<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">loss of interest or pleasure in activities you enjoy<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\"><a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/causes-of-exhaustion.htm\">fatigue<\/a> or lethargy\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\"><a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/low-self-worth.htm\">feeling worthless<\/a> or <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/why-do-i-feel-guilty-all-the-time.htm\">guilty<\/a><\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/cant-focus-might-one-psychological-health-conditions.htm\"><span style=\"font-weight: 400;\">finding it difficult to concentrate<\/span><\/a><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">changes in weight and appetite<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/sleeplessness.htm\"><span style=\"font-weight: 400;\">sleep problems<\/span><\/a><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/managing-suicidal-thoughts.htm\"><span style=\"font-weight: 400;\">suicidal thoughts<\/span><\/a> (depression with psychosis raises your risk of this symptom).<\/li>\n<\/ul>\n<p><strong>Then, in addition to the above symptoms, <\/strong>you&#8217;ll experience <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/flashback-vs-hallucination-vs-delusion.htm\">delusions and\/or hallucinations<\/a>.<\/p>\n<p><strong>Delusions are not sensory, they are beliefs. And ones that <\/strong>aren&#8217;t based on reality, often involving <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/what-is-paranoia.htm\">paranoia<\/a>. The most common is<\/strong> hearing demoralising voices. But they can equally be seeing things that aren&#8217;t there, feeling sensations that aren&#8217;t there (for example, like someone is touching your arm), or even tastes from things you have not eaten.<\/p>\n<p><strong>Note that psychotic depression can also affect your motor skills. You might be <\/strong>more agitated than usual so that you can&#8217;t sit still, or more slowed down.<\/p>\n<h2>How is this different than schizophrenia?<\/h2>\n<p><strong>Psychosis is a set of symptoms, not a disorder<\/strong> in and of itself. Schizophrenia and major depressive disorder with psychosis are two distinctly different disorders.<\/p>\n<p><strong>T<\/strong><span style=\"font-weight: 400;\"><strong>he main symptom<\/strong> <strong>of schizophrenia is<\/strong> a marked and consistent issue with determining what is real and what isn&#8217;t.\u00a0<\/span><\/p>\n<p><strong>With depression with psychosis, the main symptom is<\/strong> severe <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/types-of-depression.htm\">depression<\/a>, to the point you lose touch with reality.\u00a0 <span style=\"color: #333333;\">So with with psychotic depression, you\u2019ll have depression first. Then you will experience hallucinations and delusions that are depressive and focus on themes like<a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/why-do-i-feel-hopeless.htm\"> hopelessness<\/a> and <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/overcoming-fear-of-failure.htm\">failure<\/a>.<\/span><\/p>\n<p><span style=\"font-weight: 400;\"><strong>Psychosis in schizophrenia, on the other hand, can involve impossible or bizarre themes<\/strong> that don\u2019t connect with a mood state. You might think that someone is staring at you because they want to steal your phone.<\/span><\/p>\n<blockquote><p><span style=\"color: #333399;\">You are more likely to realise that your hallucinations and delusions are not real if you have depression with psychosis than if you have schizophrenia, where it can be harder to differentiate.\u00a0<\/span><\/p><\/blockquote>\n<div id=\"attachment_125871\" style=\"width: 277px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-125871\" class=\"size-medium wp-image-125871\" src=\"https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-guilherme-rossi-1995550-267x400.jpg\" alt=\"psychosis and depression\" width=\"267\" height=\"400\" srcset=\"https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-guilherme-rossi-1995550-267x400.jpg 267w, https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-guilherme-rossi-1995550-400x600.jpg 400w, https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-guilherme-rossi-1995550-600x900.jpg 600w, https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-guilherme-rossi-1995550.jpg 709w\" sizes=\"auto, (max-width: 267px) 100vw, 267px\" \/><p id=\"caption-attachment-125871\" class=\"wp-caption-text\">photo by Guilherme Rossi for Pexels<\/p><\/div>\n<p><strong>And while schizophrenia can involve low moods,<\/strong> given that it can deeply affect your <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/building-confidence.htm\">confidence<\/a> and <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/a-sense-of-self-who-am-i.htm\">sense of self?<\/a> Depression is not a requirement of this mental health disorder.<\/p>\n<p><strong>Note that if someone with schizophrenia also continuously suffers severe depression<\/strong>, they then might be diagnosed with &#8216;<a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/what-is-schizoaffective-disorder.htm\">schizoaffective disorder<\/a>&#8216; instead.<\/p>\n<h2>Am I at risk?<\/h2>\n<p><span style=\"font-weight: 400;\"><strong>Your genetics can put you at higher risk, especially if<\/strong> you\u2019ve had a close family member who\u2019s had depression with psychosis.\u00a0<\/span><\/p>\n<p>And <a href=\"https:\/\/www.nhs.uk\/mental-health\/conditions\/psychotic-depression\/\">according to the NHS<\/a>, if you experienced<span style=\"font-weight: 400;\"><a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/what-are-adverse-childhood-experiences-aces.htm\"> childhood adversity<\/a>, such as <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/how-can-i-remember-childhood-trauma-abuse.htm\">trauma<\/a> from <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/what-is-child-sexual-abuse-why-your-definition-might-need-updating.htm\">sexual abuse<\/a> or physical abuse, you are at a higher risk.\u00a0<\/span><\/p>\n<h2><b>Causes of psychotic depression<\/b><\/h2>\n<p><span style=\"font-weight: 400;\"><strong>Genetics has a role to play. There\u2019s a theory that you need to inherit a<\/strong> combination of genes which would give rise to depression with psychosis. You could have different sets of genes, one set responsible for your depression symptoms and the other for your psychotic symptoms. This would make you susceptible to depression, psychosis, or both.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\"><strong>As for depression, there are many factors. For example, according to<\/strong> <a href=\"https:\/\/www.nature.com\/articles\/srep30187\">this study,<\/a><\/span><span style=\"font-weight: 400;\"> high levels of the hormone cortisol from increased stress contributes to the development of depression. Depression is also highly related to sleep problems, and, of course, challenging life circumstances. But it&#8217;s not known exactly why if two people go through the same tough experience, one might develop depression and the other doesn&#8217;t.\u00a0<\/span><\/p>\n<h2>How is depression with psychosis diagnosed?<\/h2>\n<p><span style=\"font-weight: 400;\"><strong>You would need to <\/strong><a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/talk-to-a-doctor-about-anxiety.htm\">see your GP<\/a> or a <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/psychiatrist-vs-therapist.htm\">psychiatrist<\/a>. If your GP thinks you have psychosis, or that it&#8217;s actually schizophrenia or schizoaffective disorder, you would be referred on to a psychiatrist in any case.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\"><strong>Many people are hesitant to<\/strong> share their psychotic experiences, so for doctors, diagnosing them with psychotic depression can be challenging.<\/span><\/p>\n<p><strong>And sometimes, although again it&#8217;s less likely than if you<\/strong> actually had schizophrenia, you might not realise that you are acting psychotic. To you, your delusions and hallucinations might seem very real, and only those around you might feel there is an issue.<\/p>\n<p><strong>If you are already severely depressed and people around you<\/strong> comment you are acting strange, it might be important to listen to what they have to say and ask for clear examples.<\/p>\n<h2>What treatment will I be offered?<\/h2>\n<p><span style=\"font-weight: 400;\"><strong>Recommended treatment for depression with psychosis in the UK tends to be<\/strong> a mix of medication, talk therapies, and social support. Cognitive behavioural therapy<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3775151\"> (CBT) has been found to help with psychosis<\/a>.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\"><strong>In some cases, if you don&#8217;t respond to medication, you might be offered<\/strong> the choice to try electroconvulsive therapy<\/span><span style=\"font-weight: 400;\"> (ECT). <\/span><span style=\"font-weight: 400;\">The procedure involves the transmission of electrical current to the brain via the scalp for a short period. It is performed under general anaesthesia.\u00a0<\/span><\/p>\n<h2>When to seek medical help<\/h2>\n<p><span style=\"font-weight: 400;\"><strong>Again, depression with psychosis brings a higher risk of<\/strong> suicidal thinking than just depression on its own. So it&#8217;s important you take your symptoms seriously. <\/span><\/p>\n<p><span style=\"font-weight: 400;\"><strong>You should speak to a medical or mental health professional as soon as you can<\/strong> if you\u2019re experiencing any of the symptoms of psychotic depression.<\/span><\/p>\n<p><span style=\"font-weight: 400;\"><strong>If you or someone you know is having suicidal thoughts or thoughts of self-harm, note that<\/strong> trained volunteers are available 24 hours a day, 7 days a week through the <\/span><a href=\"https:\/\/www.samaritans.org\/\"><span style=\"font-weight: 400;\">Samaritans<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0\u00a0<\/span><\/p>\n<p><strong><span style=\"color: #800080;\">Need help with severe depression?
