{"id":126501,"date":"2022-10-04T11:00:26","date_gmt":"2022-10-04T10:00:26","guid":{"rendered":"https:\/\/www.harleytherapy.co.uk\/counselling\/?p=126501"},"modified":"2023-03-22T13:23:00","modified_gmt":"2023-03-22T13:23:00","slug":"black-mental-health","status":"publish","type":"post","link":"https:\/\/www.harleytherapy.co.uk\/counselling\/black-mental-health.htm","title":{"rendered":"Black Mental Health &#8211; It&#8217;s Time We Started Talking"},"content":{"rendered":"<div id=\"attachment_126502\" style=\"width: 390px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-126502\" class=\"wp-image-126502 \" src=\"https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-joshua-mcknight-1191503-400x266.jpg\" alt=\"black mental health\" width=\"380\" height=\"253\" srcset=\"https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-joshua-mcknight-1191503-400x266.jpg 400w, https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-joshua-mcknight-1191503-500x333.jpg 500w, https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-joshua-mcknight-1191503-180x120.jpg 180w, https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-joshua-mcknight-1191503-600x400.jpg 600w, https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-joshua-mcknight-1191503.jpg 709w\" sizes=\"auto, (max-width: 380px) 100vw, 380px\" \/><p id=\"caption-attachment-126502\" class=\"wp-caption-text\">photo by Joshua McKnight for Pexels<\/p><\/div>\n<p><span class=\"s1\" style=\"color: #333399;\">by Claudia Cole<\/span><\/p>\n<p><span class=\"s1\">What are the issues we all need to start talking more about when it comes to black mental health?\u00a0<\/span><\/p>\n<h2>Higher need, yet less referrals<\/h2>\n<p><em>Research shows that the Black community suffers from more mental health challenges, yet black clients are less likely to be referred on for treatment and support.\u00a0<\/em><\/p>\n<p class=\"p2\"><span class=\"s1\">According to a government survey, <a href=\"https:\/\/www.ethnicity-facts-figures.service.gov.uk\/health\/mental-health\/detentions-under-the-mental-health-act\/latest\"><span class=\"s2\">Black adults are almost five times more likely to be detained<\/span><\/a> under the Mental Health Act than any other ethnic group.<\/span><\/p>\n<p class=\"p2\"><span class=\"s1\"><a href=\"https:\/\/www.ethnicity-facts-figures.service.gov.uk\/health\/mental-health\/adults-experiencing-common-mental-disorders\/latest#by-ethnicity-and-sex\"><span class=\"s2\">Black women in the UK experienced a higher rate of common mental<\/span><\/a> health conditions, such as <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/talk-to-a-doctor-about-anxiety.htm\">anxiety and depression.<\/a> And Black men are ten times more likely to experience <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/depression-with-psychosis.htm\">psychosis<\/a>.<\/span><\/p>\n<p><strong>And yet other research shows, for example, that B<\/strong><span class=\"s1\"><strong>lack patients from<\/strong> African and Caribbean backgrounds with<a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/bipolar-disorder.htm\"> bipolar disorder<\/a> or psychosis being less likely to be referred for <a href=\"https:\/\/www.harleytherapy.co.uk\/cognitive-behavioural-therapy-london.htm\">cognitive behavioural therapy (CBT)<\/a> than white patients.<\/span><\/p>\n<h2>Racism and mental health<\/h2>\n<p class=\"p2\"><span class=\"s1\"><strong>Often, race-related stressors play a fundamental part in a<\/strong> Black person&#8217;s reality. This can negatively impact their mental health. <\/span><\/p>\n<blockquote>\n<p class=\"p2\"><span class=\"s1\" style=\"color: #800080;\"><a style=\"color: #800080;\" href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4580597\/\"><span class=\"s2\">Research<\/span><\/a> shows that regular exposure to racism is associated with symptoms of depression and anxiety.<\/span><\/p>\n<\/blockquote>\n<p class=\"p2\"><span class=\"s1\"><strong> This can overlap with other factors, such as<\/strong> social disadvantages, increasing the risk of mental health problems. And yet all this is not often taken into account when mental health professionals are formulating treatment plans.\u00a0<\/span><\/p>\n<h2 class=\"p3\"><span class=\"s1\">How are the experience and outcomes of care different? <\/span><\/h2>\n<div id=\"attachment_126505\" style=\"width: 285px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-126505\" class=\"size-medium wp-image-126505\" src=\"https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-david-kuko-2743754-275x400.jpg\" alt=\"black mental health\" width=\"275\" height=\"400\" srcset=\"https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-david-kuko-2743754-275x400.jpg 275w, https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-david-kuko-2743754-413x600.jpg 413w, https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-david-kuko-2743754.jpg 709w\" sizes=\"auto, (max-width: 275px) 100vw, 275px\" \/><p id=\"caption-attachment-126505\" class=\"wp-caption-text\">photo by David Kuko for Pexels<\/p><\/div>\n<p class=\"p2\"><span class=\"s1\"><strong>Even when receiving mental health care, the overall experience for<\/strong> Black and ethnic minority groups was considered more negative than positive.\u00a0<\/span><\/p>\n<blockquote>\n<p class=\"p2\"><span class=\"s1\"><span style=\"color: #333399;\">In a <a style=\"color: #333399;\" href=\"https:\/\/www.manchester.ac.uk\/discover\/news\/services\/downloadfile.php?f=rho-rapid-review-final-report-v.7.pdf&amp;uid=1139877&amp;hash=8c1f63a90532ae3ede061168dad317e77b0447d6\"><span class=\"s2\">review<\/span><\/a> commissioned by the NHS Race and Health Observatory, there was evidence of Black individuals receiving harsher treatment in inpatient wards. This often involved using physical restraints or being put in seclusion, making patients feel unsafe<\/span>.<\/span><\/p>\n<\/blockquote>\n<h2 class=\"p3\"><span class=\"s1\">Barriers to black mental health care<\/span><\/h2>\n<p class=\"p2\"><span class=\"s1\">While we all have the right to receive care and treatment for our mental health, Black and minority ethnic people may face additional struggles that can affect getting the help they need. W<\/span><span class=\"s1\">hat are these potential barriers?<\/span><\/p>\n<h3><span class=\"s1\" style=\"color: #333399;\">1. Inability to recognise mental health problems.<\/span><\/h3>\n<p class=\"p2\"><span class=\"s1\">Some individuals are unable or unwilling to recognise symptoms of mental health problems. It may not be something that&#8217;s openly spoken about in their community due to the <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/handling-depression-stigma.htm\">stigma<\/a> attached, or because of cultural beliefs. As a result, seeking support may be delayed, often to just before the point of <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/what-is-a-nervous-breakdown.htm\">crisis<\/a>.<\/span><\/p>\n<h3><span class=\"s1\" style=\"color: #333399;\">2. Racial stereotyping and discrimination.<\/span><\/h3>\n<p class=\"p2\"><span class=\"s1\">Ongoing racial stereotypes could influence attitudes and treatment decisions towards people from minority backgrounds. <\/span><\/p>\n<p class=\"p2\" style=\"text-align: left;\"><span class=\"s1\">For example, Black women have been widely depicted as &#8216;being strong.&#8217; This perception can lead to their feelings being dismissed or not taken seriously by professionals. Similarly, Black men face a stereotype of their own. They have historically been depicted as <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/myths-about-domestic-violence.htm\">violent<\/a> and dangerous, leading to the use of more restrictive care.\u00a0<\/span><\/p>\n<h3><span class=\"s1\" style=\"color: #333399;\">3. Lack of trust for the system.\u00a0<\/span><\/h3>\n<p class=\"p2\"><span class=\"s1\">As highlighted in the<a href=\"https:\/\/www.nhsrho.org\/publications\/ethnic-inequalities-in-healthcare-a-rapid-evidence-review\/\"> Ethnic Equalities in Healthcare review<\/a>, some individuals distrust mental health professionals over the fear of racist treatment. This can lead to large delays or even avoidance in seeking mental health support.\u00a0<\/span><\/p>\n<h3><span class=\"s1\" style=\"color: #333399;\">4. Lack of context around black mental health. <\/span><\/h3>\n<p class=\"p2\"><span class=\"s1\">In some cases, the experiences and concerns of black individuals are overlooked when being assessed and diagnosed as there is more emphasis on symptoms than context.<\/span><\/p>\n<p class=\"p2\"><span class=\"s1\">For instance, antidepressants might be a recommended option for treating symptoms of depression.\u00a0 <\/span><span class=\"s1\" style=\"color: #333333;\">Even though it can be beneficial, some may not find this form of treatment useful alone, as it doesn&#8217;t help if on a daily basis they are facing <a style=\"color: #333333;\" href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/money-stress.htm\">poverty<\/a> and\/or<a style=\"color: #333333;\" href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/righteous-indignation.htm\"> racial discrimination<\/a>.\u00a0<\/span><\/p>\n<h2 class=\"p5\"><span class=\"s1\">Why therapy might be seen as a problem<\/span><\/h2>\n<div id=\"attachment_126504\" style=\"width: 395px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-126504\" class=\" wp-image-126504\" src=\"https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-alex-green-5699424-400x266.jpg\" alt=\"black mental health\" width=\"385\" height=\"256\" srcset=\"https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-alex-green-5699424-400x266.jpg 400w, https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-alex-green-5699424-500x333.jpg 500w, https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-alex-green-5699424-180x120.jpg 180w, https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-alex-green-5699424-600x400.jpg 600w, https:\/\/www.harleytherapy.co.uk\/counselling\/wp-content\/uploads\/pexels-alex-green-5699424.jpg 709w\" sizes=\"auto, (max-width: 385px) 100vw, 385px\" \/><p id=\"caption-attachment-126504\" class=\"wp-caption-text\">photo by Alex green for Pexels<\/p><\/div>\n<blockquote>\n<p class=\"p6\"><span class=\"s1\" style=\"color: #800080;\">For many marginalised people, this lack of context can make therapy come across as a temporary fix to their very real daily problems. Or even leave them feeling they are <a style=\"color: #800080;\" href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/blame.htm\">being blamed<\/a> for having such problems.\u00a0<\/span><\/p>\n<\/blockquote>\n<p class=\"p2\"><span class=\"s4\">Psychologist and writer Dr. <a href=\"https:\/\/www.theguardian.com\/commentisfree\/2022\/sep\/06\/psychologist-devastating-lies-mental-health-problems-politics\">Sanah Ahsan<\/a> points this out in a Guardian piece on society\u2019s <a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/what-is-mental-health.htm\">flawed perspectives on mental health<\/a>. She argues <\/span><\/p>\n<blockquote>\n<p class=\"p2\"><span class=\"s4\" style=\"color: #333399;\">\u201cIt is here that we fail marginalised people the most&#8230; Black people\u2019s understandable expressions of hurt at living in a structurally racist society are too often medicalised, labelled dangerous, and met with violence under the guise of \u2018care\u2019.\u201d<\/span><\/p>\n<\/blockquote>\n<h2 class=\"p5\"><span class=\"s1\">Are mental health professionals informed?<\/span><\/h2>\n<p class=\"p2\"><span class=\"s4\"><strong>Another challenge Black people face in therapy is lack of cultural understanding<\/strong> by mental health professionals, which can interfere with the effectiveness of talk therapy.\u00a0<\/span><\/p>\n<blockquote>\n<p class=\"p2\"><span style=\"color: #333399;\"><span class=\"s1\">According to a <a style=\"color: #333399;\" href=\"https:\/\/bmjopen.bmj.com\/content\/6\/11\/e012337\"><span class=\"s2\">research<\/span><\/a><\/span><span class=\"s5\"> review<\/span><span class=\"s1\">, Black and ethnic minority people often struggle to be understood, with a few participants expressing their annoyance with having to explain &#8220;the Black experience.&#8221;<\/span><\/span><\/p>\n<\/blockquote>\n<p><span class=\"s4\"><span class=\"s1\"><strong>It does not help that if someone is offered therapy on the NHS they are not allowed to choose<\/strong> which therapist they work with but are allocated someone. While some areas of the UK have NHS branches who recognise the issue and offer a <a href=\"https:\/\/www.alliancepsychology.com\/nhsadult\/black-minority-ethnic-group\/\">Black and minority ethnic service (BAME)<\/a>, this is not always the case. And not <\/span><\/span><span class=\"s4\"><span class=\"s1\">everyone can afford the luxury of <a href=\"https:\/\/www.harleytherapy.co.uk\/\">private therapy<\/a> where they can control who their therapist is.<\/span><\/span><\/p>\n<h2 class=\"p3\"><span class=\"s1\">What is cultural competence, and can it help?<\/span><\/h2>\n<p class=\"p2\"><span class=\"s1\"><strong>Cultural competence is the ability to acknowledge and understand cultural differences,<\/strong> allowing therapists to meet the needs of their patients of all backgrounds. It is an approach to care where race, ethnicity, and beliefs are respectfully taken into account. <\/span><span class=\"s6\">A culturally competent therapist will be familiar with these subjects and can help address any issues in a way that&#8217;s comfortable for you.<\/span><\/p>\n<p class=\"p2\"><span class=\"s1\"><a href=\"https:\/\/www.harleytherapy.co.uk\/counselling\/what-is-the-therapeutic-alliance.htm\">Building a relationship with your therapist<\/a> and feeling understood is important in therapy. The topic of race is delicate yet necessary. Seeing a cultural competence therapist can make it easier, allowing you to become more open about your feelings and experiences. <\/span><\/p>\n<h2 class=\"p3\"><span class=\"s1\">What should I do if the support is not working out for me?<\/span><\/h2>\n<p class=\"p2\"><span class=\"s1\"><strong>When it comes to treatment and support, it&#8217;s important to know that your concerns deserve to be heard.<\/strong> \u00a0<\/span><span class=\"s1\">If you&#8217;re having difficulty developing a good relationship with your therapist, or their approach doesn&#8217;t suit you, it&#8217;s worth expressing your thoughts so they can improve and adapt their service. Otherwise, talk to your referrer, as there might be other options that could be more effective.<\/span><\/p>\n<p class=\"p2\"><span class=\"s1\"><strong>What matters is seeking support over suffering in silence.<\/strong> This does not always have to look like one-on-one therapy.
